SFN 323, JOBS Status Change form, or the automated computer system generated JOBS Status form may be used by the TANF Eligibility Worker and the JOBS Employment Contractor to communicate and document pertinent information regarding a JOBS participant. The JOBS Status Change Form may be used:
- When a permanent record is needed to document approval for adjustments in supportive services;
- To verify whether or not a Pay After Performance individual met the work participation requirements;
- To notify the JOBS Employment Contractor that an application has been denied or a case has been closed; and
- To report employment or other changes.
